NOTE:

My office is located Uptown at 6221 South Claiborne Ave., Suite 201 New Orleans, LA 70125. I am on the 2nd floor of a 3 story building located in a shopping center across the street from the Tulane University baseball stadium and upstairs from the UPS Store and Bayou Hot Wings. There is ample free parking in the lot as well as on the street if the lot is temporarily full during the lunch rush for the restaurants downstairs. 

Once you enter the building, punch in my 3-digit access code (201) on the key pad and I will grant you access to the elevator. Once you get off the elevator on the 2nd floor, go around the corner to the office on the right at the end of the hall.

Dr. Costa is also licensed in Utah and is an approved Florida telehealth provider providing confidential virtual sessions via Zoom.
